H1N1新型流感疫情發展及個人平時之預防措施

注意流感患者之危險徵兆

發現新型流感患者或疑似患者出現以下危險徵兆均應及早就醫積極治療:呼吸急促、呼吸困難、嘴唇發紫或變藍、血痰、胸痛、意識不清、高燒三天、低血壓。

H1N1新型流感防疫速訊

從各國之臨床流行病學資料重點說明,病例之感染年齡以10-19歲為最多約佔36%,其次為0-9歲約佔25%,50歲以上者僅佔6%左右;50-80%重症病例的健康狀況是罹患有慢性疾病如:氣喘、肺部疾病、心血管疾病、糖尿病、免疫功能不良、神經性疾病及懷孕等,另外還發現肥胖也是一個罹患重症的危險因子。還有,從死亡個案的資料分析得知,從發病至死亡之期間,約為4天到14天不等(中位數為9天)

H1N1新流感的傳染力很強,但是絕大多數是屬於輕症

H1N1新流感病毒的傳播方式,目前公認主要是藉由飛沫接觸等途徑而傳播感染,至於空氣傳播方式大多是在特別的情況下發生,如:執行氣管內插管、氣管鏡檢查等醫療行為。因此,為防範感染H1N1新流感最重要的是有呼吸道症狀的人,應戴上外科口罩,在沒有口罩時,用手帕、衛生紙擋住口鼻,衣服、衣袖也可暫時應急,防止咳出的病毒感染到他人,而一般健康的民眾則於平時的生活環境中,現階段是不需要佩戴口罩,只要勤洗手維持良好的手部衛生習慣並落實呼吸道咳嗽禮節即可

H1N1新流感的傳染力很強,但是絕大多數是屬於輕症上學、上班的民眾,如有發燒、類流感等症狀時,至少要居家休息數天,大部分病人是可以恢復健康的。然而,症狀一旦有變化,如出現:呼吸短促、發紺、呼吸困難、血痰或有顏色痰液、胸痛、意識改變、高燒持續3天以上、低血壓等危險症狀時,一定要立即就醫接受臨床處置,克流感抗病毒藥物的給予亦是一種有效的醫療處置措施。

Friends,

Thanks to media hype about H1N1, several people who trust me have either approached or called me to advice. The hype in media about the utility of face masks and N95 respirators as a tool for general protection against H1N1 can't be deplored enough. Yesterday, a friend who listened wanted me to write down briefly what I advised so that he could tell others in similar words. Hence this short email to friends whom I have advised recently (and others whom I haven't yet).

Please realize that this is not an official advice, especially the one about face masks
or N95. Most N95 respirators are designed to filter 95% particulates of 0.3µ, while the
size of H1N1 virus is about 0.1µ. Hence, dependence on N95 to protect against H1N1
is like protecting against rain with an umbrella made of mosquito net.

大部分N95口罩的设计是过滤95% 的0.3µ微粒,,而H1N1病毒的大小是0.1µ 左右.所以,靠N95口罩防H1N1就像用蚊帐网做的雨伞挡雨.

Tamiflu does not kill but prevents H1N1 from further proliferation till the virus limits
itself in about 1-2 weeks (its natural cycle). H1N1, like other Influenza A viruses, only
infects the upper respiratory tract and proliferates (only) there. The only portals of
entry are the nostrils and mouth/ throat/eyes. In a global epidemic of this nature, it's
almost impossible not coming into contact with H1N1 in spite of all precautions.

克流感不杀H1N1 ,它只防止病毒扩散大约1-2周(它的自然周期).

H1N1,跟其他A型流感的病毒一样,只感染上呼吸道,并在那里扩散.病毒的唯一入口是口鼻喉.在这种本性的全球流行病疫中,幾乎所有防範努力都无法避免与H1N1接触.

Contact with H1N1 is not so much of a problem as proliferation is.

接触H1N1不像H1N1扩散问题那末严重.

While you are still healthy and not showing any symptoms of H1N1 infection, in order
to oprevent proliferation, aggravation of symptoms and development of secondary
infections, some very simple steps - not fully highlighted in most official communications - can be practiced (instead of focusing on how to stock N95 or Tamiflu):

为了防止扩散和第二次被感染的症状加剧与发展,在你还健康与没有任何H1N1

感染症状时,可以做下面的几个非常简单步骤(而不是聚焦于N95与克流感的囤积):

1. Frequent hand-washing (well highlighted in all official communications).

 常洗手

2. "Hands-off-the-face" approach. Resist all temptations to touch any part of face
(unless you want to eat or bathe).

 「手离脸」方式.(除了要吃東西与洗澡外),抗拒所有用手接触脸部任何部分的冲动.



3. Gargle twice a day with warm salt water (use Listerine if you don't trust salt).
H1N1 takes 2-3 days after initial infection in the throat/ nasal cavity to proliferate
and show characteristic symptoms. Simple gargling prevents proliferation. In a way,
gargling with salt water has the same effect on a healthy individual that Tamiflu has on
an infected one. Don't underestimate this simple, inexpensive and powerful preventative
method.

每天用温盐水(可用Listerine漱口水替代)漱口二次.首次感染,二/三天后H1N1在後鼻腔开始扩散与显示特定症状.漱口防止扩散.健康人的漱口与病人用克流感的效果是一样的.



4. Similar to 3 above, clean your nostrils at least once every day with warm salt
water. Not everybody may be good at Jala Neti or Sutra Neti (very good Yoga asanas to
clean nasal cavities), but blowing the nose hard once a day and swabbing both nostrils
with cotton buds dipped in warm salt water is very effective in bringing down viral
population.

每天用棉签沾温盐水清鼻孔至少一次与用鼻子用力呼气一次,对降低滤过性毒菌数量是很有效的.

5. Boost your natural immunity with foods that are rich in Vitamin C (Amla and other
citrus fruits). If you have to supplement with Vitamin C tablets, make sure that it also
has Zinc to boost absorption.

吃富维他命C的食物以增强免疫力.如果需要用维他命C药片补充,確認它含锌(以加强吸收).

6. Drink as much of warm liquids as you can. Drinking warm liquids has the same
effect as gargling, but in the reverse direction. They wash off proliferating viruses
from the throat into the stomach where they cannot survive, proliferate or do any harm.

尽量多喝温水.喝水会将扩散病毒从喉洗到胃里.病毒一旦到胃裡就不能生存、扩散、与造成伤害.


7. All these are simple ways to prevent, within means of most households, and
certainly much less painful than to wait in long queues outside public hospitals.

所有这些都是简单的预防方法,在每个家庭都做得到的,比在公立医院外面排长龙的痛苦要少很多.
